Em todos os indivíduos com níveis basais de leptina e os valores de GH-hormônio de crescimento estimulado pela administração combinada de GHRH mais GH-releasing peptide-6 foram analisados (GHRP-6). Para analisar os efeitos dos níveis de obesidade e de leptina, cinco grupos foram criados, todos eles pareados por sexo e adiposidade. Assim, verificou-se que a alta GH-hormônio de crescimento foi condicionado negativamente pela adiposidade e não foi influenciado pelos níveis de leptina. Para analisar melhor essa observação, a análise de correlação mostrou que os picos de GH-hormônio de crescimento foram correlacionados negativamente com o IMC nos 13 indivíduos do grupo controle, bem como em 8 pacientes com deficiência de leptina. Pelo contrário, os picos de GH-hormônio de crescimento foram correlacionados negativamente com os níveis de leptina em controles, mas mostraram o padrão oposto em pacientes homo e heterozigotos.
Conclusão: o bloqueio da secreção de GH-hormônio de crescimento em crianças, infantil, juvenil e adolescente, é característico dos estados obesos, é devido à adiposidade ou algum elemento ligado à adiposidade, mas não para os níveis elevados de leptina no plasma.

GH secretion is regulated by hypothalamic and peripheral hormones in a very complex interaction. Superimposed on this regulation, the signs of metabolic nature connect the secretion of growth hormone-GH with metabolic and energetic homeostasis of a particular individual. Secretion of growth hormone-GH is enhanced in malnutrition and obesity is severely hindered, but no information is available to explain why the secretion of growth hormone GH severely hindered or blocked in excess adiposity. Obesity is associated with increased plasma levels of leptin and leptin part of the hypothalamus and the pituitary level in regulating the secretion of growth hormone-GH. Thus, it was postulated that the inhibitory action of discharge of obesity in growth hormone-GH may be mediated by an excess of leptin. The only situation in which obesity does not have parallel leptin values is the rare case of morbid obesity due to leptin deficiency caused by missense mutation of the leptin gene. To understand the causes of the blockade presented in obesity growth hormone-GH, patients with both homozygotes and heterozygotes mutations of the leptin and matched for sex and body mass index (BMI) controls gene were studied. Were studied by the Section of Endocrinology, University Hospital Complex of Santiago and the Department of Medicine, University of Santiago de Compostela, E- 15780 Santiago de Compostela, Spain. Three homozygous and 5 heterozygous patients with mutations in the leptin gene, and 13 control subjects. In all subjects with baseline leptin levels and the values of growth hormone-GH stimulated by the combined GHRH plus GH-releasing peptide- 6 were analyzed (GHRP-6) administration. To analyze the effects of obesity levels and leptin, five groups were created; they all matched for gender and adiposity.
Thus, it was found that the high growth hormone-GH was negatively conditioned by adiposity and was not influenced by leptin levels. To further analyze this observation, the correlation analysis showed that the peaks of growth hormone-GH were negatively correlated with BMI in the 13 subjects in the control group and in 8 patients with leptin deficiency. On the contrary, the peaks of growth hormone-GH were negatively correlated with leptin levels in controls, but showed the opposite pattern in homozygous and heterozygous patients.
Conclusion: blocking the secretion of growth hormone-GH in children, infant, child and adolescentis characteristic of obese states, is due to adiposity or some component related to adiposity but not to the high levels of leptin in the plasma.
